I am also not aware of any reports out of the box. I agree with you Dale that it is something you would expect out of the box. We are using a couple of custom Grids for this that Advoco made for us. I have attached a screenshot of one of the grids
I don't see a canned report that is going to give you what you are looking for here. A custom report will certainly get you the data you are looking for.
